Subject: – APPLICATION FOR GRANT OF STUDY LEAVE
R/Sir,
I am directed to refer to your department letter No. AO/E&SE/4-20/SL/Karak dated 04-10-2019 on the subject cited above and to state that the following documents are required for grant of study leave.
- Surety Bond after completion of his/her Ph-D/Course, the applicant will return the same station/job to resume his duty and will serve the department for five years.
- Attested copy of first appointment order.
- Course relevancy certificate by the Administrative Department.
- Certificate to the effect that the officer has not avail study leave previously.
- No objection certificate by the Administrative Department
- Call admission letter by the concern institute.
- Leave application form in original dully authenticated by the District Accounts Officer.
- Service length certificate
- Exact date of commencement of study leave
- Duration of course
- Original admission receipt deposited for the subject course
- Last academic qualification certificate/documents dully attested.
